      At the present stage of development the aim of the health system is the organization of qualita¬tive models of infrastructure and management schemes. In the given aspect the most urgent health problem is to build a high-quality system of medical care. The article summarizes the results of the situational analysis of medical activities and evaluation of diagnostic services at the present stage in the conditions of state financing and transition to full cost accounting in the Republic of Tajikistan. Analysis of normative-legal documents showed that of 2866 institutions of primary health care 3,8% of them are represented by the district and city health centers, 26,5% of rural health centers, 10,8% in the specialized outpatient centers, 58,9 per cent – houses of health.     • The clinical and immunological features of onychomycosis in elderly and the senile age

      From among 45 patients at the age of 60-85 (men – 36, women – 9) with onychomycosis 23 (51.1%) had distal and lateral form, 5 (11.1%) -superficial and white, 6 (13.3%) – proximal and hyponychial, 11 – total and dystrophic. All patients with onychomycosis had various forms of the athlete’s foot. The cell immunity decreased (fall of CD3 and CD8) and the humoral one increased (raise of IgM, IgG and the circulating immune complexes).

    • IMPORTANCE OF DRAINAGE FOR PURULENT SPACES IN THE COMPLEX TREATMENT OF PATIENTS WITH PUTREFACTIVE-NECROTIC PHLEGMON OF THE MAXILLOFACIAL REGION

      The article presents the results of treatment of 24 patients with putrid green-necrotic abscess the bottom of the cavity of mouth. Analysis of the carried out research showed that the expansion unit from alloys nickelide-titanium have obvious advantages to the traditional types of drainage, namely: improvement of visual access to the wound and their widespread separation, which leads to the improvement of aeration, which have a great importance in the conditions of mixed and anaerobic microflora.
      All of the above mentioned activities with the use of expansion of devices allowed speeding up the time of cleansing and healing of wounds on average by 3-5 days and proceeding to more early medical and social rehabilitation.

      Studying of a current of pregnancy and sorts at women with an insufficient actual food has shown high relative density, late hestos, threates, anomalies of patrimonial activity, a trauma of soft patrimonial ways, an asphyxia of newborns. Lactation infringement it is diagnosed for every seventh woman in childbirth. Studying of the reasons гипогалактии has shown them: mediko- social, connected with complications of pregnancy and the sorts, caused by affective and personal infringements.

      The conducted research is a substantiation for system engineering of preventive, medical and rehabilitation actions for correction of a diet of an actual food of pregnant women and women in childbirth taking into account mediko- social factors that will allow to reduce number of complications during pregnancy and sorts, frequency and weight reduction milk allocation, to improve quality of life of feeding mothers.

      One of the most urgent problems in the structure of modern somatic pathology remains endo¬toxicosis, which is a basic component of the formation of the severity of the General condition. The article presents the results of analysis of toxic blood components 206 patients endotoxemia developed in the result of acute renal failure, peritonitis and septicemia in comparison with the figures of 20 healthy donors. Material for the study served as the blood of patients aged from 17 to 62 years, of which men made up 59,3% of women – 40,3%. The study used biochemical, gel-chromatography and infrared spectroscopic methods of diagnostics indicators token to reflect the level of intoxication. Found that the toxicity index in patients endotoxemia increases by more than 700%, compared with the control.
