Abstract
To compare the clinical outcomes of different types of endoscopic surgical interventions in patients with chronic recurrent maxillary sinusitis depending on the ineffectiveness of previously used treatment methods.
We observed 21 patients aged 20 to 47 years with recurrent purulent maxillary sinusitis from 2020 to 2025. The study included anamnestic, clinical and laboratory, otorhinolaryngological, and radiological methods. The patients were divided into two groups. In the first group, endoscopic surgery for sinus sanitation was performed by enlarging the natural ostium, and further postoperative care of the sinus cavity was carried out through it. In the second group, in addition to enlarging the natural ostium, an additional infraturbinal opening was created and a polyethylene tube was inserted using the “through-drainage” technique for further irrigation of the sinus with antiseptic solutions.
The study included 15 (71.4%) men and 6 (28.6%) women. The mean age of the patients was 35.2 years. Group 1 included 11 (52.4%) patients, and Group 2 included 10 (47.6%) patients. Endoscopic examination of the middle nasal meatus revealed slight narrowing of the natural ostium due to synechiae formation and mucosal edema, as well as a small amount of mucous discharge in the sinus cavity, in 3 (27.3%) patients in Group 1 at 6 months and in 1 (9.1%) patient at 9 months after treatment. In Group 2, narrowing of the ostium due to mucosal thickening was observed in only 1 (10%) patient at 9 months after surgery. Newly formed ostial narrowing was eliminated endoscopically in all patients.
The treatment of choice for chronic recurrent maxillary sinusitis caused by persistent stenosis of the natural ostium is surgical management using modern techniques with maximal preservation of the physiological function of the mucosa. In cases where patients had undergone repeated surgical treatment without positive clinical dynamics, the proposed “through-drainage” method helped prevent recurrent relapses.
