Abstract

Intracerebral hemorrhage (ICH) remains the most severe form of acute cerebrovascular accident (stroke), with extremely high rates of mortality and disability. This review discusses current epidemiological trends, pathogenetic mechanisms, neuroimaging capabilities, and therapeutic strategies for hypertensive intracerebral hematomas (HIH). Special attention is given to the evolution of surgical tactics – from open craniotomy to minimally invasive interventions – as well as to patient selection criteria for different treatment approaches. The literature search was conducted in PubMed, MEDLINE, Scopus, Web of Science, CyberLeninka, and eLIBRARY databases for the period 2018-2025 using the following keywords: “intracerebral hemorrhage,” “hypertensive intracerebral hematomas,” “neuroimaging,” “conservative therapy,” “minimally invasive surgery.”
